To be eligible to register at the surgery, you must live within the surgery's catchment area.
New Patient Registration
In order to register you will need to provide details of your previous GP and the address you were living at when you were registered there. We will also need your present address. (Please note, if you are a member of a college, the college is now your present address). If regsitering in person you will be required to complete a registration form, a new patient questionnaire and a data sharing choice form.
We would encourage you to book an appointment with our practice nurse for a new patient health check within four weeks of joining the surgery.
Your registration will be with the surgery rather than with a particular doctor, although you will be given a 'named GP'. Dr Drury will be the named GP for patients whose surname begins with the letters A - G, Dr MacKeith will be the named GP for patients whose surname begins with the letters H - O , and Dr Kirkaldy will be the named GP for patients whose surname begins with the letters P - Z. You can change your named GP if you wish - just let us know. Named GPs have been introduced to ensure all patients have a dedicated individual who is responsible for their care. However you are always free to choose whichever doctor you would like to see.
It is very important that you inform us if you change your name, address or telephone number. Your health may be at risk if we need to contact you and all our contact details are out of date.
Please be aware, you may be asked for proof of UK residency and entitlement to NHS Services.
Registration
It is now possible to register online -- simply complete our new patient registration form by pressing the button below.
We now allow patients to conduct many of their dealings with us online and you can even view your own medical record online too. To register for this service you also need to complete a form. You can complete this form online and then print it, sign it and bring it with you to the surgery. We also need to see two forms of ID before we can grant you online access.
