Sharing your data
Your patient record is held securely and confidentially on our practice computer system. Two schemes (one local and one national) are now in place to enable electronic sharing of certain elements of your medical record, should you require treatment in another NHS healthcare setting such as an out of county A&E Department or a local hospital. You must give, or withold, your consent to both of these systems before your registration can be competed. This can be done by completing and returning the choice form.
(patient leaflets and website information are available about both of these data sharing systems - the Summary Care Record and the Oxfordshire Care Summary, e.g. SCR Patient Leaflet and SCR website, e.g. OCS Patient Leaflet and OCS website).
We also participate in another local initiative which makes it possible for health professionals working from the 'out-of-hours' health centres in Oxford and Abingdon to view (with your permission) your GP medical record if treating you when we are closed. We have provided more information about this record sharing arrangement and details of how to opt out, and the organisers - OxFed - have also produced this leaflet.
